Our work brings together the NHS, public health, general practice, dentists, community pharmacy, ophthalmology, social prescribers, community hubs, local food growers, health professionals, voluntary organisations, academics, community groups and people who are passionate about health creation. Funded by Cornwall and Isles of Scilly ICB, we have been working across our county, to accelerate awareness, inspire action and bring about transformative change.

Based at Volunteer Cornwall, we are a small team working within the NHS primary care sector supporting transformational change for health creation, climate and nature.

Read our Atlas to find out how we are supporting primary care across Cornwall and the Isles of Scilly to deliver multiple co-benefits for our colleagues, our patients, our communities and our planet.
